Massage Therapy Permits
Establishments
A permit is required by any person, firm, partnership, association or corporation who wishes to operate a massage therapy establishment. The application maybe be obtained below or from the Codes Division. The annual fee for the establishment is $200.
Therapists
Each person employed to perform massage therapy services in a massage establishment is required to obtain a Massage Therapist's permit. The application may be obtained below or from the Codes Division. The annual fee for the establishment is $25.
